Key features

Cap white, flat-topped like a box, intensely chrome-yellowing when touched, especially at the margin and at the base of the stem. Flesh in the stem base lemon yellow when cut; unpleasant smell: phenolic, of ink or of a hospital; causes persistent vomiting and diarrhea.
